Parent Consent Form – Health & Counseling Services

A new state law requires Snyder ISD to have parent or guardian consent before our nurses or counseling staff can provide services to your child.

We ask all families to complete the consent form as soon as possible. This allows us to continue providing the same care and support your child has always received. Without your consent, our staff will only be able to help in life-threatening emergencies.

Physical Health Services May Include

Provided by school nurses or athletic trainers:

  • Health assessments and routine first aid for minor injuries

  • Administration of over-the-counter medications per district policy

  • Monitoring chronic health conditions (e.g., asthma, diabetes)

  • State-mandated screenings (vision, hearing, scoliosis)

  • Support during illness or physical symptoms at school

  • Heat illness prevention and injury support for student-athletes

  • Standing orders from the District’s Medical Director

Mental Health & Counseling Services May Include

Provided by (by school counselors or mental health staff)

  • Short-term individual counseling or check-ins

  • Student-success needs assessments (academic-related, not diagnostic)

  • Social-emotional skill-building or small group counseling

  • Support during emotional distress or crisis situations

  • Behavioral observations and problem-solving

  • Referrals to external providers, if needed (with additional consent)

What This Consent Does Not Include

This form does not authorize:

  • Formal medical or psychological diagnoses

  • Administration of prescription medications without authorization

  • Ongoing therapy or treatment that requires parental notification by law

  • These services require separate written consent, except in emergencies or situations legally exempt.
